You can find the serial number on the back of your FreeStyle Libre reader in the grey square. The 13 figure serial number will follow a format similar to this example: JCMX000-K0000 [4 letters, 3 numbers - 1 letter, 4 numbers]

How it works

For users who are unable to use the FreeStyle LibreLinkØ app, you may be eligible* for a complimentary FreeStyle Libre 2 reader. Firstly, we will need you to answer a few questions and have your FreeStyle Libre reader handy.

Replacement is subject to Freestyle Libre reader serial number check. 

Once the form is submitted, it will be checked for eligibility. If you are eligible for a complimentary Freestyle Libre 2 reader, it will be dispatched within 14 working days.
